Community Connectors Service
Slough CVS's Community Connectors programme is an innovative initiative launched on July 1st, 2023 as part of our new contract for Voluntary and Community Sector Support Services from Slough Borough Council. Our mission is to empower Slough residents to begin a journey towards improved wellbeing and self-sufficiency. The project's vision is to enhance the lives of residents, patients, and carers in Slough, focusing on enabling individuals to develop their strengths and establish long lasting connections to their interests, communities, and a network of wellbeing, care, and support services.
Referrals to Community Connectors
If you know of any Slough residents whose health and wellbeing could be improved through community-led services, please refer them to the Community Connectors for one-to-one support using the link: Refer to Community Connectors
We support clients over the phone by exploring a range of options to identify the services that best meet their needs. We can also update you about the support we have provided.
Top tips to remember when referring to Community Connectors
1.We do not deliver services directly or carry out home visits.
Our primary role is to signpost and connect Slough residents to local social groups, activities and community services that support overall health and wellbeing.
2. We can support wider social issues when linked to wellbeing
Where appropriate, we can connect residents to additional services if their wider challenges impact their wellbeing. These may include:
- Food poverty
- Employment issues
- Mental health
3. We cannot accept referrals for housing issues or from social prescribers.
We cannot accept referrals where housing or homelessness is the main or only concern. We also do not accept referrals where a resident is receiving support from a social prescriber.
4. Referrers can contact us for signposting advice
If a full referral isn’t appropriate, professionals are welcome to email us with queries. We can provide guidance and information that can be shared directly with the resident.
5.Community Connector support is not a substitute for statutory care
We do not undertake safeguarding, risk assessments, or case‑holding. Our signposting offer does not replace statutory or clinical services.
