During MaDE Week, the new Slough CVS (SCVS) Community Connectors Project, led by Kinder Bassi, proactively initiated connections with hospitals and engaged with in-patients preparing for discharge. This proactive approach aims to support smooth transitions, improve efficiency, and provide necessary resources for patients as they leave the hospital.

SCVS was assigned the Jubilee and Acute Medical wards at Upton Hospital and focused on providing essential information to patients, including Cost-of-Living details, befriending support services, and guidance on accessing the Wellbeing for Slough Community Directory of activities and services.
